Traditional Benefits

Health Plan

Larson Companies offers a fully-insured health plan which has two plan options.  The plan does not require referral to receive the highest benefits under the plan.  At the time you need care, simply choose a provider to receive the in-network or out-of-network benefit level.  The premium for this insurance is paid by both employer and employee.

 

Dental Plan

Larson Companies offers a dental plan where you have a choice for your dental care, including participating and non-participating providers. The premium for this insurance is paid by both employer and employee.

 

Vision Plan

Larson Companies offers vision benefits under a program which offers a comprehensive vision care insurance plan for you and your family’s vision care needs, including coverage for participating and non-participating providers.

 

Employee and Dependent Life Insurance

Participants in the Health Plan are automatically provided basic term life insurance coverage.  This benefit provides a death benefit to your survivor(s) if you die while covered by the plan.  The premium for basic life insurance is completely paid by your employer.

 

Optional Supplemental Insurance is separate from the Basic Life Insurance coverage you may be provided by participating in the Health Plan.  This life insurance would provide you with personal life insurance coverage  for yourself and your dependents.

 

Optional Accidental Death and Dismemberment (AD&D) is available with the supplemental life insurance in an equal amount.  If you, or your dependent, dies in an accident, AD&D pays an additional benefit equal to the supplemental life insurance coverage.

 

Short - Term Disability

Participants in the Health Plan are automatically provided STD insurance coverage.  This benefit provides you coverage of 66.7% of your weekly pay. The premium for STD is completely paid by your employer.

 

Long - Term Disability

The LTD plan provides continuing income if you become totally disabled and are unable to work.  This benefit provides you coverage of 60% of your monthly earnings.  

 

Health Care Spending Account

Spending accounts allow you to set aside money to pay for eligible expenses with tax-free dollars.  Use the health care spending account to cover the cost of health, dental, vision and hearing expenses which are not covered under an insurance plan for you and your dependents. 

 

Dependent Care Flexible Spending Account

Use this account to cover the cost of dependent care with tax-free dollars while you work.  You may use this for expenses for the care of a child under the age of 13 or a disabled spouse, child or parent.
